EXPERIENCE: Describe the particular situation or event.
At approximately 10:30 hours on Sunday, August 01, 2010, the M/V Sealth (ON D662478), carrying out her normal operating schedule on the San Juan Island interisland route, was transiting from the Shaw Island terminal to the Orcas Island ferry terminal in good weather and visibility. During the arrival process at the Orcas Island Ferry terminal the vessel experienced a “hard landing” with the dock. No crew injuries were sustained. Two reported minor passenger injuries were experienced. There was no damage to the vessel. There was minor wingwall support structural damage that resulted in no disruption to terminal operations with a repair cost estimated to be approximately $17,000.00.

LESSON: Describe what knowledge was gained from the experience.
The proximate cause of the accident was the failure of the Mate, who was in charge of the navigation watch, to follow established SMS procedures for the Issaquah class vessel propulsion engagement process. The largest contributing factor to the incident was the Mate's lack of familiarization with the vessel and route procedures and inadequate oversight by the Master with a new mate. Unless otherwise noted, the following narrative is based on the statements of the Master and Mate, compared with electronic tracking and mechanical data logger information. Variations are duly noted. No deficiencies or mechanical problems were discovered in the propulsion system of the vessel. All navigational and electronic equipment were functioning normally. The audible alarm that sounds in the event of propulsion control failure was functional but did not sound. The data logger logs indicate that the vessel propulsion engagement process was normal for an Issaquah class vessel.

RECOMMENDATION: Describe how the knowledge gained can be used.
Future training for Mates should emphasize the navigation watch procedures in the Safety Management System. Additionally, future training for Masters should include scenario based training with emphasis on proper oversight of mates.
