EXPERIENCE: Describe the particular situation or event.
At approximately 10:23 a.m. hours on Sunday, August 22, 2010, the M/V Evergreen State (ON 268732), carrying out her normal operating schedule on the San Juan Islands inter-island route, was departing the Shaw Island terminal in good weather and visibility when the vessel did an emergency back down and impacted the left wing wall of the berth (viewed from land) with the stern of the ship with sufficient force to cause moderate damage to the vessel and cosmetic damage to the berth wing wall structure. No crew or passenger injuries were experienced. Damage to the ship was contained to the foremost portion of the main car deck and supporting structure completely above the waterline. No environmental impact was experienced. Immediately after the incident, the crew surveyed damage; upon finding no breach in the hull, the vessel continued on her normal operation until she could be repaired at dockside availability. The USCG was notified of the allision at approximately 11:30 a.m. that day by the Operations Watch Supervisor. Vessel repairs were completed by Friday, September 17, 2010 and, following sea trials, the vessel resumed service that day. Total cost of vessel repairs was approximately $50,000.00. According to the Terminal Engineering Department of Washington State Ferries, repairs to the berth and wing wall are not necessary.

LESSON: Describe what knowledge was gained from the experience.
The proximate cause of the accident was the failure to follow established SMS procedures for Vessel Arrival, Vessel Departure, and Deck Department Standing Orders. The largest contributing factor to the incident was the lack of direction to the Bridge Team by the Master. Unless otherwise noted, the following narrative is based on the collective Bridge Team’s statements. Variations or disputes are duly noted. No deficiencies or mechanical problems were discovered in the propulsion system of the vessel. All navigational and electronic equipment were functioning normally.

RECOMMENDATION: Describe how the knowledge gained can be used.
Future training for Masters should include scenario based training with emphasis on proper oversight of the navigation team. The training should reinforce the principles of Bridge Team Management.
