EXPERIENCE: Describe the particular situation or event.
At approximately 18:55 hours on Saturday, March 27, 2010, the M/V Hyak (ON 508160), carrying out their normal operating schedule on the Anacortes/San Juan Islands route, was making an approach to the Lopez terminal on Lopez Island when the vessel impacted the left wingwall (viewed from the shore) with the starboard bow of End No. 1 with sufficient force to cause damage and break timbers in the wingwall structure. No crew injuries were experienced. No passenger injuries were sustained. No damage was sustained by the ship. No environmental impact was experienced. Following the incident, the ferry was successfully moored in the berth and traffic was discharged uneventfully. Following inspection of the ship and terminal, it was determined that scheduled service could be resumed. Cost of actual repairs to the wingwall and associated structures was approximately $9,000 plus mobilization costs of approximately $28,000, bringing the total cost of repairs to approximately $37,000.

LESSON: Describe what knowledge was gained from the experience.
The proximate cause of the accident was a shiphandling misjudgment complicated by opposing wind and current effects. Contributing to the accident was a human factor of an inexperienced mate in charge of the vessel movement without close supervision by the Master. There is evidence that an inappropriate command on the Hyak’s Engine Order Telegraph (EOT) may have caused a momentary delay in the response to a command for astern propulsion. If true, this error may have resulted in a slight delay in astern propulsion and may have contributed to the allision with the wingwall.

RECOMMENDATION: Describe how the knowledge gained can be used.
1. That all WSF Masters should be counseled that regardless of who is in charge of the navigational watch, the Master’s responsibility and accountability for the safe operations and maneuvering of the vessel under his/her charge is absolute. 2. That whenever possible in a hard landing situation, an announcement should be made to all passengers and crew to “brace for possible impact” to hopefully minimize any injuries caused by unfamiliar ship motions. Safety Management System procedures should be reviewed for modification as necessary with any appropriate additions. 3. That the state of tide and tidal currents should be recognized and fully considered by all watchkeepers particularly those handling the ship. An appropriate communication to all watch standers should be developed. 4. That this Safety investigation be referred for a Human Relations investigation concerning the actions of the navigation team, particularly the Master and Mate responsibilities. 5. That Washington State Ferries develop and promulgate a system by which “Lessons Learned” from Marine Incidents such as this can be shared with operational crews.
